Registration with the Information Commissioner's Office in combination with European General Data Protection Regulations oblige me to provide information regarding how the data I need to ask you for is protected.

As a counsellor and supervisor in private practice, I collect name, address, phone number(s) email address, date of birth and GP details when we agree to work together. This information is stored securely as a paper record. Email addresses of current clients are stored securely on my computer and anonymised phone numbers of current clients are stored securely on my phone. Paper records are kept under lock and key and phone numbers are PIN-protected. I use your phone number, email or address to contact you about appointments and to reply to you when you contact me. I do not share your data with a third party unless you ask me to or unless I am obliged to by a court of law. In this unlikely event, I would seek to contact you first. All information is securely destroyed three years after our work together has ended.

In the event of my death or incapacity causing me to be unable to contact you myself, my professional executor would do so, using the above information which she would then ensure was securely destroyed.
